Public facing data ingest mechanism and help

It would be a good idea to have some easy to use mechanism for people to request their data be included in LAMHDI. Would this simply be a request form? or could/should we have them use some kind of template to help generate what we'd need for Disco? Something like the NIF "add resource" functionality but tailored for actual data ingest.
