“i want my data in lamhdi” registration service

In order for an external group to request a new data resource gets added into the LAMHDI system, we need a web form for them to request their data service. Maybe this can generate JIRA ticket(s).

This probably should be linked too pretty obviously from the main site, and should be the answer to a FAQ.
